This application should ONLY be completed by GOVERNMENT OF RWANDA EMPLOYEES who are applying for their GOVERNMENT TRAVEL CARD for the FIRST TIME or applying for a REPLACEMENT CARD.  

You will required to show your National ID or International Passport when picking up your card. You will also be trained on how the card works, and you will be given a Cardholder Handbook.
